USAU Rostering

Rostering requirements for MA HS league:
These are the requirements for your team to be considered eligible for the spring season. Any games played before these conditions are met will not be considered in determining eligibility/qualification for the State Championships. Descriptions of how to get a USAU ID and how to create a team roster follow.
- Individual players:
- Current USAU youth membership ($30) and waiver, or have paid the $7 “one time event fee” (For eligibility to play in States, the $30 membership is required. The one-time fee is not sufficient.)
Membership/Waiver form - Give your coach the Medical Authorization Form (this gives a medical professional permission to provide medical assistance even if your parents are not there to give consent). Coaches/chaperones must be in possession of these forms at every game.
Medical Authorization form

- Chaperones (can be the Coach): complete a background check, as detailed on the USAU site:

- Teams:
- Must have a roster of at least 7 greenlighted players by April 9th.
- Complete the 2013 MA HS Ultimate League Registration form.
How to get a USAU ID for new players:
- Go to the New Member Page and complete the form. Your USAU login information will be emailed to you in a couple hours.
- Check your email and follow the link to verify your application. Login to the USAU Member Login page, where you will find your USAU ID number.
How to create a team roster (The coach or team Captain does this):
- Log in to your USAU ID and click on Online Rostering.
- Read the directions, then click on the Create an Event Roster link
- Read those directions, then click Continue with Roster Creation
- Click Sanctioned and Insured Events
- Click 2013S MA HS LEAGUE
- Follow the process as it leads you.
If this seems like a lot of work – you’re right, it is! However, now you are done for the year. If your team participates in any other USAU sanctioned or sponsored event (including States), you can simply have the roster copied over to that event.
YOU HAVE UNTIL MONDAY, APRIL 8th TO SUBMIT AT LEAST A PARTIAL ROSTER (7 PLAYERS). ANY GAMES PLAYED WITH FEWER THAN 7 GREEN-LIT PLAYERS IS A VIOLATION OF THE SANCTIONING REQUIREMENTS, AND WILL NOT COUNT TOWARD THE SIX GAME MINIMUM REQUIRED FOR STATES ELIGIBILITY.
